Asuris Clarity does not require members to visit a preferred care provider or obtain referrals for specialists. Select a health savings account (HSA), Comprehensive Preferred Plan (PPO), Catastrophic Preferred Plan (PPO), or Asuris Clarity 50/70/80 Plans.
The Asuris Clarity 50/70/80 Plans include:

• Preventive Care Benefits – included with Clarity 70 and Clarity 80.
• Office Visits – members have 100% coverage (after copay, before deductible) for unlimited office visits.
• Drugs – copay covers generic prescription drugs (before meeting the deductible).
• Asuris Advantages – discounts include LASIK surgery, contact lenses, hearing aids, contact lenses, and more.

Additionally, Asuris Clarity is available at affordable pricing for students and children, age 24 and younger, as well as self-employed individuals. Medicare options include the MedAdvantage Plan, Part D Prescription Drug Coverage, and Traditional Medicare Supplemental plans.
Asuris Clarity also provides plans for employers – regardless of size – including:
