Horizon Healthcare may be better known as the New Jersey affiliation of Blue Cross Blue Shield, but it has a reputation for providing quality service irrespective of its name. Horizon has a number of plans available for individuals and families on a budget, including:
- Horizon HMO: This is a managed plan with no deductible or co-insurance, and is a popular choice for those who cannot afford high deductible plans. Comprehensive coverage is provided by a co-pay, including unlimited home health, maternity, emergency room and in-patient hospital coverage. Prescription drug coverage is available at a co-insurance of 50%.
- Horizon Basic Plan A/50: This is a freedom-of-choice plan that works like a PPO. Enrollees choose any doctor and any deductible they wish, ranging between $1,000-5,000, and co-insurance amounts vary from 50%-70%. Pregnancy, emergency care and outpatient services are provided, as well as prescription drug coverage.
- Horizon MSA: This plan is intended for use with a high deductible plan. It is similar to a Health Savings Account. Both families and individuals are eligible to enroll in a Medical Savings Account.
- Horizon Basic/Essential EPO/EPO plus: Members must choose a doctor in their network for coverage, but they don't have to stay with a single physician, as there are no referrals or PCPs required.
In addition, Horizon offers their members access to discounted dental and vision care, and alternative medicine choices. Horizon Blue Cross Blue Shield is currently rated B++(very good) by A.M. Best.
