Mountain State BCBS' Flagship Product
SuperBlue Plus allows members to see non-network healthcare providers as well as use network providers. Using network providers will save members the most money. There are several deductibles from which to choose, $250, $500, $1000, $2500, and $5000. Coinsurance for network providers is 20% and for non-network providers, 40%. Maximum out of pocket is $5,000 per year when using network providers.
There is no maximum out of pocket when using non-network providers. Benefits include hospitalization, well-children, nursing care, and physician services. Prescription drugs are not subject to a deductible and have a 50% coinsurance. If a member is in an accident, the member is completely covered without a deductible to up to $500, including ambulance services. Members can travel throughout the United States and be able to find and use network healthcare providers.
