Members may choose from the following options:

• Cox HealthPlans HMO – The HMO option provides comprehensive coverage for in-network providers. Coverage for specialists is available when the primary care physician provides a referral.
• Cox HealthPlans PPO – The PPO option allows members to visit in-network providers or those out-of-network, providing them with the greatest freedom of choice. The service provided by out-of-network physicians, however, will be at a greater cost.
• Out of Area (PHCS) – Cox Health Systems also services areas outside of their 25-county service area, by partnering with Private Health Care Systems (PHCS). This partnership allows access to the leading PPO network in the United States, including 450,000 providers and 4,000 facilities nationwide.

Cox Health Systems aims to “be the health plan of choice for employers and individuals in Southwest Missouri by achieving and maintaining organizational excellence in risk management, customer service, effective decision making, and control of fixed costs.” To best meet these goals, Cox offers a variety of plans with differing co-payments and deductibles:

• Group Plans – can be customized depending on budget and employee needs; a variety of plans for both HMO and PPO are available.
• Individual Plans – offer quality healthcare at affordable prices; plans are available for self-employed individuals, those looking for more affordable dependent coverage, or for children.
• Kids First – a plan designed specifically for children, with coverage for ages six months to 18 years. This plan is very affordable as it covers only a child.
