Group Health Cooperative's insurance offerings include the following products:  

• 500 Deductible Plan: offers 20% coinsurance after a $500 deductible is met for an individual.  Preventative services have 20% coinsurance without having to meet the dividend.  Prescription drugs require a $20 copay for generics and a $40 copay for branded.  Other services covered include hospitalization and maternity benefits. 


• 1500 Deductible Plan: is a catastrophic health plan.  Coinsurance is 80% after meeting the deductible, except for preventative services. You will not have to meet your deductible in order to start using your 20% coinsurance when using preventative care services.  Benefits include hospitalization and office visits.

• 2500 Deductible Plan: does not cover prescription drugs or maternity care.  Coinsurance is 50% after meeting your deductible with the exception of preventative care services, which are not subject to meeting the dividend.


• 5000 Deductible Plan: covers office visits and hospital care after you meet your $5,000 deductible with a 30% coinsurance.  Preventative care is covered with a 30% coinsurance without having to meet your deductible.


• 500 Welcome Plan: has a $500 deductible.  Preventative care and the first four office visits per year have a 50% coinsurance without having to meet a deductible. After meeting your deductible, services such as hospital care and maternity care have a 50% coinsurance. Prescription drugs are covered with a small copay.


• The 1000 Welcome Plan: has a $1,000 deductible.  Coverage is similar to the 500 welcome plan except maternity services and prescription drugs are not covered.


• Group Health Cooperative offers four different HSA plans:
  
  
  1. 1500 Individual Plan
  2. 2500 Individual Plan
  3. 3000 Family Plan
  4. 5000 Family Plan 

The deductible does not have to be met when using preventative care services.  Coinsurance for preventative care, office visits, and hospital care is 80%.
