Individual Plans are HMOs offering a choice in eight deductibles.  Deductibles choices are between $150 and $5,000 for individuals.  The higher your deductible, the lower your premiums will be.  Out of pocket expenses, or the maximum amount of money that you will have to pay on a yearly basis, range from $850 for the $150 deduction to $8,000 for the $5,000 deduction.  Covered services with no coinsurance requirements are prenatal care and well-child care.  Services that have 20% coinsurance after meeting the deductible include eye exams, physical exams, office visits, and hospitalization.  Prescription drugs are also covered, but may require a separate deductible depending on the plan that you choose.  Members must see healthcare providers from a network.

Empower HSA Plan is a high deductible plan which offers you low premiums.  An HSA allows you to use pre-tax dollars to pay qualified medical expenses.  In many cases, this can save you money on your taxes.  Any money left in the HSA account at the end of the year is simply rolled into the next year.  Many people use HSA accounts not only to pay their medical bills, but as a long term savings account.  Empower HSA Plans with $1,200 and $2,000 deductibles for individuals have 20% coinsurance after deductibles has been met.  Plans that have $2,600 and $5,000 deductibles do not have any coinsurance after deductibles are met.   Services include office visits, emergency room care, outpatient care, prescription drugs, and hospitalization.
