• $1,500 Deductible Plan is an HMO where members are required to use doctors and healthcare providers from within the Kaiser Permanente network. There is a separate $250 deductible for brand name prescription drugs and for outpatient surgery.   Annual out of pocket, or the maximum amount of money an individual can spend on a yearly basis, for this plan is $3,500.  After the deductible has been met many of the services, including routine care and eye exams, require a $30 copay. Other services included in this plan are hospitalization and emergency coverage.

• $250 Deductible Plan is an HMO with an annual out of pocket of $2,500 for individuals.  There is a separate $250 deductible for brand name prescription drugs.  Many of the services covered, such as routine appointments, eye exams, and hearing tests, require a $20 copay after the deductible has been met. • $50 Copay Plan, an HMO, does not have any deductible to meet.  Maximum out of pocket is $3,500 per year for an individual plan.  Many services including, office visits, eye exams, and hearing tests have a $50 copay.  Other services in this plan include hospitalization and emergency coverage.  Prescription drugs are not covered.

• $25 Copay Plan offers most coverage without a deductible.  Copays for many services such as office visits, eye exams, and hearing tests, have a $25 copay.  Brand name drugs have a  $250 deductible.  Other services covered include hospitalization and outpatient services.  This plan is also an HMO.
