LifeWise Health Plan of Arizona
Sadly, LifeWise Health Plan of Arizona no longer offers coverage in the state. In February of 2009, the company announced that it would discontinue service in Arizona due to inadequate membership numbers. LifeWise AZ Health Insurance Details
Although LifeWise plans are not available in the state, we thought you would benefit from looking at its benefits to compare them with other providers in Arizona. For this reason, LifeWise offers the following products:
- Essential 20 offered two choices in deductibles, $2,500 for network providers and $5,000 for non-network providers or $5,000 for network providers and $10,000 for non-network providers on individual plans. Coinsurance was 20% for network providers and 50% for non-network healthcare providers for most services after meeting deductibles. Coverage included office visits, prescription drugs, and hospitalization.
- Passport Plan offered five choices in deductibles ranging from $500 for network providers and $1,000 for non-network providers to $5,000 for network providers and $10,000 for non-network providers. Prescription drugs had a separate deductible. Coinsurance was 20% for network providers and 50% for non-network healthcare providers for most services after meeting deductibles. Services covered included office visits, hospitalization, outpatient surgery, and hospice care.

- HSA 20 was an HSA plan that allowed members to pay qualified medical expenses using pretax dollars. This enabled many members to save on their taxes. Health Savings Accounts could have also been used as a long term savings plan, as any money or accrued interest left at the end of the year rolled into the next year. This encouraged members to use their healthcare dollars wisely. HSA 20 had three choices in deductibles. Network and non-network deductibles had to be met separately. After deductibles were met, coinsurance was 20% for network providers and 50% for non-network providers for most services. Coverage included prescription drugs, office visits, and hospitalization.
- HSA Plan was another HSA option offered by LifeWise. Members did not have to pay any coinsurance for network services after meeting a $5,000 deductible. Non-network coinsurance was 50% after meeting a $10,000 deductible. Coverage included prescription drugs, hospitalization, and office visits.
