• LifeWise Plus is a PPO with a choice of five deductibles ranging from $500 to $7,500 for individuals when using network providers. Non-network providers have a separate deduction. Coinsurance after meeting deductibles is 20% for network providers and 40% for non-network healthcare providers.  Members pay a $20 copay for office visits without having to meet their deductible when using network providers.  Coverage also includes prescription drugs and hospitalization.

• LifeWise Preferred is a PPO which allows members to choose from five deductibles from $500 to $7,500 when using network providers.  Non-network providers have a separate deduction.  Coinsurance after meeting the deductibles is 20% when seeing network providers and 40% when using non-network providers.  Preferred plans cover services including prescription drugs, doctor visits, and hospitalization.

• LifeWise Value plans offer four deductible choices from $1,000 to $7,500 for individuals who use network healthcare providers.  There are separate deductibles for non-network providers.  Coinsurance is 30% for network providers and 50% for non-network medical providers after deductions have been met.  Coverage includes hospitalization and office visits.  These plans have prescription drugs as an optional benefit.

• LifeWise HSA PPO plans are high deductible, low premium plans that offer members tax savings.  Approved medical expenditures are paid from Health Savings Accounts to which members have contributed pre-tax dollars.  LifeWise PPO has deductibles from $2.500 to $5,000.  These plans cover prescription drugs, hospitalization, and office visits.

• LifeWise HSA Choice individual members enjoy a $2,500 deductible and a 20% coinsurance for all providers after the deductible has been met for most services.  Coverage includes hospitalization, office visits, and prescription drugs.
