• Essentials 25 is a PPO, allowing members to use either network or non-network healthcare provides.  Essentials 25 has a choice of two deductibles, $1,500 for network and $3,000 for non-network providers or $2,500 for network and $5,000 for non network providers.  Coinsurance for network providers is 25% and for non-network providers, 50% on most services after meeting the deductibles.  Services covered include office visits and hospitalization, both inpatient and outpatient.

• Essentials 50 has the same deductibles and the same type of coverage as does Essentials 25, but has a 50% coinsurance after the deductibles have been met for most services for both network and non-network providers.

• Passport 20 has a choice of two deductible plans, $1,000 for network providers and $2,000 for non-network providers or $1,500 for network providers and $3,000 for non-network providers.  Coinsurance for most services is 20% for network providers and 50% for non-network providers after deductibles are met.  Coverage includes prescription drugs, hospitalization, and office visits.

• Passport 30 has the same two choices of deductibles as Passport 20 another choice a $500 deductible for network and a $1,000 deductible for non-network healthcare providers.  Coinsurance on most services is 30% for network providers and 50% for non-network providers.  The coverage is similar to Passport 20.

• Passport 50 includes one more deductible plan than does Passport 30, a $2,000 deductible for network providers with a $4,000 deductible for non-network healthcare providers.  Coinsurance is usually 50% for both network and non-network providers after deductibles have been met.  Coverage includes hospitalization, office visits and prescription drugs.

• LifeWise Health Plan of Washington offers two HSA plans, LifeWise HSA Plus, and LifeWise HSA.  HSA plans offer members high deductibles and low premiums coupled with the opportunity to save on taxes.  Members contribute pre-tax dollars to a Health Savings Account.  They use this account to pay qualified medical expenses.  LifeWise HSA Plus and LifeWise HSA members are able to apply both network and non-network charges to their deductibles.
