- Classic One 90/70: This PPO plan has deductibles rangng from $250-$5,000. The plan involves a 90% co-insurance. Office visits, prescription drugs and hospitalization are included in the plan.
- Classic One 80/60: This plan is identical to the Classic One 90/70, with one exception: The 80/60 plan pays an 80% copay. In-patient and out-patient services, as well as preventive treatment are covered.
- Midwest Security HSA: This plan is for those who need a high-deductible health insurance policy to go along with their HSA. Unlike other plan options, the HSA and High-deductible plan complement each other, allowing for comprehensive care at an affordale price.
