Each plan requires that a primary care provider handle the member's routine care and preventive visits, as well as make referrals to specialists or hospital care. Different family members may select a different primary care provider, so not everyone in the family must use the same primary care provider. Neighborhood Health Plan offers members the choice of providers practicing at over 250 primary care sites.

Preventive/Routine Care

• Physical exams – Generally, this refers to routine check-ups
• Vaccinations – I ncluding allergy shots, flu shots, and childhood vaccinations
• Eye care – For routine eye exams, Neighborhood Health Plan does not require a referral
• OB/GYN – Includes routine exams, maternity care, family planning, prenatal and postnatal care

Diagnostic and treatment services are also covered, including specialty visits, emergency services, hospital care, medical equipment, lab tests and home health care.
