• Consumer Choice Plans – Members can choose either Health Savings Accounts (HSAs) or Health Reimbursement Arrangements (HRAs).

• Network Plan – Members visit participating network providers; plan does not include office visit copay.
• Network Copay Plan – Pay only the copay amount when visiting a participating provider.
• Hospital-Only Network Plan – Includes hospital networks that participate in the network; useful for members without a local physician network.
• Indemnity Plan – Members are free to choose any doctor or hospital, within or outside of the network.
